Forgot a couple of things, when you mount the cradle brackets ( sorry no pic ) make sure the distance betweem the two will fit between the front frame rails...the front frame rails are narrower then the rear frame rails. If you have a pivot arm that's 24" long you'll want a 21" heavy duty tarp strap,  see strap in my fist reply.

Harbor Freight has some good deals on wheels and a heavier duty trailer jack that might work better than the light duty ones.

The heavy duty one still needs supports welded on, unless you're only going to use it on the rear. The axle tubes will bend.

  Thanks for the taking the time to measure, label and take pictures!  This will be a huge help.  What size tubing did you use?  What guage or thickness?  Did you run a solid axle through the tubing to mount the wheels to or just weld a short axle onto the end of the tubing?
